Registration Forms and Documents

Youth Summer Camp Registration Forms

The following forms are required for attending camp. After the form is completed, please be sure to sign it and mail it to camp at least 2 weeks before the start date of your camp session.

*North Dakota residents can go to the ND Department of Health website and fill out a request form. The immunization record will be emailed to you.

If your child has diabetes, asthma, or an anaphylactic response to an allergy, please complete the appropriate form and attach it to his or her health form:

Camp Scholarship Information

It is our goal to make it possible for EVERY camper to be able to attend camp. If you are in need of a camp scholarship, please complete the application form and mail it to the camp. Many churches and WELCA groups offer assistance as well. Please check with them about scholarships too.
